Patients with advanced, life-limiting illnesses can greatly benefit from hospice care. In fact, the Centers for Medicare & Medicaid Services (CMS) encourages providers to make early referrals for hospice care, as it:

May extend the patient’s life longer than it would be with conventional care.

Reduces risk of complications with grief.

Allows the patient and family time to communicate about needs and wishes.

Ensures that the patient’s needs are heard and honored.

Please let us know how we can help.

Harmony Hospice specializes in working in tandem with PCPs and other healthcare providers to create a comprehensive, personalized care plan for patients and their families.

As a community-based program, we offer our services in homes, hospitals, assisted living centers, long-term care units, and other settings.

We are also pleased to offer healthcare providers a number of resources to assist them with referrals, billing, determining eligibility, pain management, end-stage indicators, and discussing end-of-life topics with patients and loved ones.

There are a number of valuable resources available with further information about hospice and palliative care.
